Request to Retest

Retests are allowed for all unit quizzes as necessary.  Any scores of a "C" or lower should be considered for retakes.  All retake quizzes must be taken within two weeks of the original assessment.  Students will be allowed no more than 3 retakes each quarter.

The form below should be printed and completed in full to document the time and effort you put forth to improve your performance on the assessment.
